Form 3921 generally reports the grant date, exercise date, exercise price, exercise-date fair market value, and shares transferred. Those are important inputs, but the exercise spread is not the amount of AMT owed. The full return determines whether AMT applies.
Build a full-year regular-tax and AMT projection
A useful projection includes the household's wages, withholding, business and investment income, gains, deductions, credits, prior payments, and other alternative minimum tax items. Form 6251 addresses the federal ISO adjustment, while Publication 505 and Form 1040-ES address withholding and estimated-tax planning.
State treatment should be calculated separately. A state may not follow the federal AMT rules, and work location, residence, source income, and a later move can change the state analysis.

Permitted wage withholding and a separate estimated payment can affect the underpayment calculation differently because withholding is generally treated under special timing rules. The available approach depends on the remaining payroll schedule and the taxpayer's facts. Use an official payment channel and preserve every confirmation.
Recalculate after a sale and preserve dual-basis records
Current Form 6251 instructions generally provide that no ISO exercise adjustment is required when the affected shares are disposed of during the same year they were exercised. The sale can still create regular-tax reporting, including a possible disqualifying disposition, and the actual result depends on the transaction records.
